This innovative plan supports members living with diabetes and ensures you have the care you need, when you need it, and at a cost that fits your budget.
Your diabetes plan includes:
$0 Copay for Select Medications:
- Regular insulins: Novolin® N, Novolin® R, Novolin® 70-30.
Rapid-acting insulins: Novolog, Novolog 70-30, Fiasp.
- Long-acting insulins and generic oral drugs: drug list.
$0 Cost for Self-Management Supplies:
- Glucometer and test strips: Roche Brand, Accu-Chek.
- Preferred diabetes supplies (pen needles, insulin syringes, alcohol swabs, urine ketone strips).
- Lancets and lancing devices.
$0 Copay for Select Medical Services:
- Diabetic eye exams.
- Diabetic foot exams.
- Diabetic related outpatient labs, including: HbA1c, Lipid panel, Metabolic panels, Urine albumin tests
- Diabetes self-management education.
- Nutritional counseling.
Free prescription delivery.
Members get free delivery on 30-,60-, and 90-day
prescription order from Health First Family Pharmacy.
